The station signed on in 1980 as KOUS-TV, a NBC affiliate. Previously, NBC had been relegated to secondary clearances on CBS affiliate KTVQ and ABC affiliate KULR-TV--as was PBS until 1984. Channel 4's sign-on made Billings one of the last markets in the nation to receive full service from all three networks.

In 1987, KOUS and KULR swapped affiliations, with KOUS taking ABC while KULR taking NBC. Ironically, KULR had been the original NBC affiliate in Billings from 1958 to 1968. In 1993, KSVI signed on and took the ABC affiliation, while KOUS picked up the Fox affiliation. The call letters changed to the current KMHT in 1995.

During the 1980s, Miles City's KYUS Channel 3 was originally a "sister station" of KOUS. KYUS was a primary NBC affiliate in the Glendive/Miles City area, which at # 212 was the smallest TV market in the United States. Since channel 4 switched to Fox, KYUS has been a satellite of KULR.

KHMT's broadcasts became digital-only, effective June 12, 2009.[1]
